About The Position

Reporting to the Primary Health Care (PHC) Manager, the Primary Health Care (PHC) Nurse is an integral member of the PHC team, providing and coordinating initial, continuing, and comprehensive nursing services to patients in the PHC site. The PHC Nurse promotes quality health care outcomes working within established standards to ensure patients/clients receive a high standard of primary health care. The PHC Nurse works alongside physicians, clinic staff, and other health care professionals to promote quality health care outcomes, while working with established standards to ensure patients/clients receive a high standard of primary health care. PHC Nurse initiates and/or participates in community development initiatives and participates in the evaluation of PHC initiatives within the site. The PHC Nurse works relatively independently and is expected to make decisions based on competent assessment knowledge and skills. The PHC Nurse plays a key role in quality improvement in the function of the clinic, primary health supports offered in the community and supports linkages between primary and secondary health care.

Requirements

  • Minimum one year of recent nursing experience in a clinic-based setting required
  • Bachelor of Science in Nursing (BScN)
  • Basic Life Support Level C Certificate (BLS-C)
  • Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)

  • Ability to act independently/work independently/make decisions autonomously
  • Knowledge in chronic disease prevention and management
  • Demonstrated clinical competency
  • Knowledge of population health approach to health promotion/prevention

  • Ability to prioritize to meet deadlines and work effectively under pressure
  • Knowledge in time management
  • Systems thinking approach to health care delivery
  • Knowledge of motivational interviewing
  • Demonstrated ability to use sound clinical judgement

  • Knowledge in LEAN management system
  • Demonstrated understanding of the prevention and management of chronic diseases
  • Knowledge in various diagnostic for cardiopulmonary disease and/or diabetes
  • Knowledge in First Nation and Metis history in Saskatchewan
  • Ability to work independently and with broader interdisciplinary teams
  • Demonstrates empathy, trust and respect in the provision of care
  • Demonstrates ability in teaching, facilitation and group activities
  • Competent critical thinking/judgement/autonomous decision making capabilities

Nice To Haves

  • Experience in case management is an asset
  • Nursing diploma is acceptable
  • Nursing experience in Chronic Disease Management is desired.
  • Experience in a community-based setting (Public Health, Home Care) is an asset.
