About The Position

Lone Tree Post Acute is seeking compassionate, skilled, and dedicated Registered Nurses (RNs) to join their team on a full-time basis. They are looking for nurses who are committed to delivering exceptional resident care while working in a collaborative, supportive, and fast-paced post-acute environment. The Registered Nurse (RN) is responsible for supervising the day-to-day nursing operations during the assigned shift while ensuring the highest standards of clinical care. Working under the direction of the Director of Nursing (DON) and Assistant Director of Nursing (ADON), the RN provides leadership to the nursing team and delivers compassionate, resident-centered care in accordance with all federal, state, and local regulations.

Requirements

  • Current and active California Registered Nurse (RN) license.
  • Current BLS/CPR certification preferred.
  • Strong clinical assessment, leadership, and communication skills.
  • Ability to work effectively both independently and as part of a multidisciplinary team.
  • Compassionate, dependable, and committed to providing outstanding resident care.

Nice To Haves

  • Previous skilled nursing, rehabilitation, or post-acute care experience is preferred.

Responsibilities

  • Supervise and coordinate nursing staff during assigned shifts.
  • Provide direct nursing care and clinical oversight to residents.
  • Administer medications, treatments, and other physician-ordered interventions.
  • Monitor resident conditions and promptly communicate changes to physicians and the interdisciplinary team.
  • Maintain accurate, timely, and thorough clinical documentation.
  • Ensure compliance with all facility policies and state and federal regulations.
  • Support, mentor, and provide guidance to Licensed Vocational Nurses (LVNs) and Certified Nursing Assistants (CNAs).
  • Participate in resident assessments, care planning, and interdisciplinary team meetings.
  • Respond effectively to resident emergencies and changes in condition.
  • Promote a safe, clean, and comfortable environment for residents, visitors, and staff.
  • Maintain resident confidentiality and comply with HIPAA standards.
