Requirements

  • Bachelor’s of Science in Nursing or Diploma in Practical Nursing
  • Registered with the College of Nurses of Ontario (CNO)
  • Current BCLS/HCP certification
  • Certificate in Wound Care (required)
  • 1–3 years of clinical experience (required)
  • Experience in complex wound care (required)
  • Experience with IV medication administration (required)
  • Familiarity with mobile work tools and EMR systems
  • Strong communication and interpersonal skills
  • Creative problem-solving and critical thinking abilities
  • Team-oriented with a client-centered approach
  • Committed to equity, inclusion, and continuous learning

Nice To Haves

  • Certificate in International Interprofessional Wound Care (IIWCC), preferred
  • Skin Wellness Associate Nurse (SWAN) certificate
  • Nurses Specialized in Wound, Ostomy and Continence (NSWOC) certificate
  • Experience in ostomy and/or continence care (preferred)
  • Experience in Palliative Care (preferred)
  • Home and community care experience (preferred)

Responsibilities

  • Provide quality and compassionate wound care/continence care to individuals in their homes.
  • Utilize the nursing process effectively, in order to provide high quality Wound and Ostomy Care for clients and/or caregivers.
  • Collaborate with the interdisciplinary health team to provide cost-effective, high-quality Wound Care, using Best Practices.
  • Apply current knowledge and research of Wound Care Protocol to practice.
  • Create, in consultation with the physician, CCAC Case Manager, collaborate with ICCP coordinators and Primary Nurse, a plan of care for patients requiring wound care in the community.
  • Initiate plan of nursing care in consultation with the client and/or caregiver and modify plan of care on an ongoing basis.
  • Promote the independence of clients and caregivers considering their needs, strengths and weaknesses.
  • Utilize available resources, facilitate clients/caregivers' problem solving, participation in decision making, and access to appropriate resources.
  • Participate in the evaluation and refinement of the Wound Care Protocol and overall program.
  • Involved in Education of other members of the SJHC team on the protocol and usage of products, providing guidance to best treatment choices.
  • Maintain regular communication with supervisor to include review of problems or unique issues and exchange feedback.
  • Participate in Bi-Weekly nursing case rounds.
  • Complete pre-visit assessments and client calls (screening, home safety etc.) in keeping with established procedures.
  • Review client medical information, medical orders and other chart information.
  • Engage in on-going skills training and development, ensuring knowledge base is up to date and accurate.
  • Develop care plans based on assessments, participating in the development of discharge plans.
  • Document the need for changes or adjustments to the care plan, frequency of visits, or other deviations from the pre-established requirements.
  • Utilize adult learning principles and establish teaching materials in order to provide health teaching to clients and families.
  • Complete required charting, visit verifications, an other required documentation, at the point of care and in an accurate and precise manner.
  • Participate in clinical policy and practice review, in collaboration with Management.
  • Participate in the evaluation and refinement of the Wound Care Protocol and overall program practices.
  • Engage in information sharing, mentoring, coaching and education of other members of the SJHC clinical care team, providing Wound Care guidance to best treatment choices.
