About The Position
The Home Health RN provides skilled nursing services to patients in their homes, supporting recovery and long-term health management. This position requires independent clinical judgment, strong organizational skills, and a focus on delivering patient-centered care.
Requirements
- Active Registered Nurse license
- Graduate of an accredited nursing program
- Current CPR/BLS certification
- Previous clinical experience required
- Strong critical thinking and assessment skills
- Ability to work independently and manage a caseload
- Reliable transportation for travel between patient homes

Responsibilities
- Assess patient health status during home visits and identify care needs
- Develop and carry out individualized care plans in accordance with physician orders
- Provide nursing care such as medication administration, wound care, and monitoring of chronic conditions
- Track patient progress and adjust care as necessary
- Maintain clear, accurate, and timely clinical documentation
- Educate patients and caregivers on treatment plans, medications, and home care techniques
- Communicate effectively with physicians and other healthcare providers
- Promote safety and infection control within the home environment
