About The Position

Activa Home Health, an AccordCare Company, is actively seeking experienced Registered Nurses for our innovative Hospital at Home Program. This opportunity is designed to attract top clinical talent for full-time and per diem roles across multiple service areas, including Vero Beach, Port St. Lucie and Stuart. We are seeking highly skilled RNs to deliver acute-level care in the home setting, supporting patients who would otherwise require hospitalization. This role is ideal for nurses who thrive in dynamic environments, deliver high-acuity care in the home while completing timely, accurate clinical documentation in Kinnser (WellSky), and are passionate about advancing patient-centered care models.

Requirements

• Active Florida Registered Nurse (RN) license
• Recent hospital-based experience required (Acute Care, ICU, ER, or Step-Down)
• Proven ability to collaborate within interdisciplinary clinical teams
• Strong critical thinking, assessment, and communication skills
• Comfort with mobile technology and electronic documentation systems
• Proof of up to date vaccinations

Nice To Haves

• Experience in high-acuity home health or complex medical care strongly preferred

Responsibilities

• Provide high-quality, acute-level nursing care in a home setting as part of the Hospital at Home program
• Collaborate with interdisciplinary teams
• Monitor patient status and respond to changes in condition with strong clinical judgment
• Deliver advanced clinical interventions while ensuring patient safety and satisfaction
• Accurately document care in electronic medical records (EMR)
• Adhere to program workflows, response expectations, and professional standards
