This role involves working collaboratively with members of the CareTeam to ensure patients receive skilled nursing care that meets their individual needs. The position focuses on assessing patients' needs, planning care, coordinating assignments, and participating in team meetings to foster a professional and productive work environment. The nurse will evaluate and document outcomes of patient care, participate in quality assurance programs, and communicate effectively with other team members. This role also serves as a resource for difficult or complex situations and participates in presenting education programs to ensure team competence. Additionally, the nurse will work with team members to monitor patient satisfaction and develop appropriate interventions, administer medication accurately, and provide direct patient care. Education is a key component, involving planning and providing education to patients, families, and caregivers based on the care plan and anticipated length of stay. The nurse will also collaborate with team members to manage the patient's hospital stay, ensuring timely completion of procedures, tests, and consults, and initiating discharge planning from day one. All patients returning home will receive a written discharge or instruction plan prior to discharge.
