About The Position
Amedisys is seeking a Registered Nurse case manager to provide home health care. The role focuses on assessing patients, coordinating and implementing care plans, supporting patient health and independence, and communicating with the clinical manager and care team. The nurse also supervises LPNs and HHAs, completes documentation, provides staff education, and participates in on-call services and continuing education.
Requirements
- At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP Clinical.
- Current RN license specific to the state or states where assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
- Candidates with less than six months of clinical RN experience must participate in the RN Intern program.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Refer patients to other disciplines based on identified needs, or document the rationale for not making a referral.
- Promote patient health and independence through teaching and appropriate rehabilitative measures, including self-care education.
- Supervise LPNs and HHAs.
- Complete documentation accurately and on time.
- Regularly communicate patient progress to the clinical manager and care team.
- Plan and provide staff education.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Participate in clinical development and continuing education programs.
- Perform other duties as assigned.
