About The Position

Generations Hospice is seeking a full-time On-Call Registered Nurse to provide after-hours hospice care throughout the Fond du Lac, Wisconsin territory. The schedule is 7 days on and 7 days off: Monday through Thursday from 4:30 p.m. to 8:00 a.m., with coverage beginning Friday at 4:30 p.m. and continuing through Monday at 8:00 a.m. Travel throughout the territory is required. The role combines independent, hands-on care with support from a remote after-hours triage team and an interdisciplinary hospice team. A $7,500 sign-on bonus is offered to eligible external or new hires.

The nurse provides compassionate, timely care to patients and support to their families during urgent or changing situations. Generations Hospice highlights lower patient-to-nurse ratios, interdisciplinary support, leadership familiar with hospice nursing, and Homecare Homebase (HCHB) for documentation. The organization provides hospice services tailored to the needs of patients and families, with care primarily delivered at home.

Requirements

• Graduate of an accredited nursing school.
• Current licensure to practice in the state of operation.
• At least one year of nursing experience.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Licensed driver with an automobile that is insured according to state or organizational requirements and in good working order.

Nice To Haves

• Three or more years of nursing experience preferred.
• Experience in hospice or a similar setting preferred.

Responsibilities

• Collaborate with the remote triage on-call team to provide hands-on after-hours care to hospice patients and supportive services to their families.
• Assess and manage urgent symptoms promptly and compassionately.
• Administer medication or adjust care according to hospice protocols or provider orders.
• Communicate changes in patient condition to the physician and obtain orders as necessary.
• Guide and support patients and families in understanding the current condition and anticipating next steps.
• When a death occurs during on-call hours, travel to the home or facility to pronounce death, support the family, contact the funeral home, and notify the hospice team.
• Complete timely documentation and communication.
