About The Position
Trinity Home Health Care is seeking an experienced Per Diem RN to join our growing team. This role is for a compassionate and dependable Registered Nurse looking for a flexible schedule and the opportunity to make a meaningful difference in patients’ lives.
Requirements
- Current Registered Nurse (RN) license
- Minimum of one year of nursing experience
- Excellent assessment and communication skills
- Reliable transportation and valid driver’s license
- Current CPR certification
- Ability to work independently and manage a flexible schedule
Nice To Haves
- Home health experience preferred
Responsibilities
- Complete Start of Care (SOC) and admission assessments
- Perform routine supervisory visits for home health aides
- Provide skilled nursing visits in patients’ homes
- Develop, implement, and update individualized care plans
- Collaborate with physicians, caregivers, and interdisciplinary team members
- Complete accurate and timely electronic documentation
- Educate patients and families on disease management, medications, and home care
