About The Position
Trinity Home Health Care is seeking an experienced Per Diem Registered Nurse (RN) to join their growing team. This role is for individuals who are compassionate and dependable, looking for a flexible schedule and the opportunity to make a meaningful difference in patients’ lives.
Requirements
- Current Registered Nurse (RN) license
- Minimum of one year of nursing experience
- Excellent assessment and communication skills
- Reliable transportation and valid driver’s license
- Current CPR certification
- Ability to work independently and manage a flexible schedule
Nice To Haves
- Home health experience preferred
Responsibilities
- Complete Start of Care (SOC) and admission assessments
- Perform routine supervisory visits for home health aides
- Provide skilled nursing visits in patients’ homes
- Develop, implement, and update individualized care plans
- Collaborate with physicians, caregivers, and interdisciplinary team members
- Complete accurate and timely electronic documentation
- Educate patients and families on disease management, medications, and home care
