About The Position

Lehigh Valley Companion Care Homes is seeking a compassionate and dedicated Registered Nurse (RN) to join our team on a PRN basis. The ideal candidate will deliver high-quality in-home and community-based care to clients, ensuring their health and well-being needs are met in a comfortable and nurturing environment.

Requirements

Current and valid Registered Nurse (RN) license in the state of Pennsylvania.
Ability to work independently and manage patient care responsibilities.
Compassionate and patient-centered approach to care.
Strong communication skills and the ability to work well with clients, families, and other healthcare professionals.
Ability to handle the physical demands of the job, including lifting and moving patients as needed
Ability to work on part time and an as-needed (PRN) schedule

Nice To Haves

Minimum of 1 year of nursing experience in home care, long-term care, or similar healthcare settings preferred.

Responsibilities

Provide skilled nursing care to patients in their homes and in the community setting.
Administer medications and monitor treatment plans as prescribed.
Perform health assessments, including vital signs, and document findings.
Assist with personal care tasks, as needed.
Support patients with mobility and physical therapy exercises to enhance their overall well-being.
Ensure patient safety and comfort through proper care and supervision.
Coordinate with family members and healthcare providers to ensure optimal care and support.
Respond to medical emergencies as needed and provide first aid as required.
Document all care provided and maintain accurate patient records to ensure comprehensive and up-to-date documentation.
