About The Position

The Registered Nurse provides assessment, care planning, and direct delivery of treatment using professional nursing care processes in a patient- and family-centered manner. The role operates in accordance with licensure and regulatory scope and standards, as well as established hospital policies, procedures, and protocols. This position is in the Med/Surg - West 3 department at Canton-Potsdam Hospital, with 37.5 scheduled hours per week on 12-hour day shifts and a weekend requirement.

Requirements

• Graduate of an accredited nursing school program.
• Current, unrestricted New York State registered professional nursing license.
• BLS certification upon hire; if the certification is not from the American Heart Association, it must be obtained within one week of the hire date.

Nice To Haves

• BSN from an accredited nursing school program.
• National certification.

Responsibilities

• Performs comprehensive patient assessments to identify patient and family needs.
• Develops and implements comprehensive plans of care involving patients and families.
• Performs and documents direct patient- and family-centered care and patient education according to nursing standards of practice.
• Identifies and responds to patient conditions to achieve positive patient outcomes.
• Performs discharge planning to ensure a safe and appropriate transition of care.
• Directs and delegates care provided by unlicensed team members.
• Provides world-class customer service to patients and families.
