About The Position

Reporting to the Primary Health Care (PHC) Manager, the Primary Health Care (PHC) Nurse is an integral member of the PHC team, providing and coordinating initial, continuing, and comprehensive nursing services to patients in the PHC site. The PHC Nurse promotes quality health care outcomes working within established standards to ensure patients/clients receive a high standard of primary health care. The PHC Nurse works alongside physicians, clinic staff, and other health care professionals to promote quality health care outcomes, while working with established standards to ensure patients/clients receive a high standard of primary health care. PHC Nurse initiates and/or participates in community development initiatives and participates in the evaluation of PHC initiatives within the site. The PHC Nurse works relatively independently and is expected to make decisions based on competent assessment knowledge and skills.

Requirements

  • Minimum one year of recent nursing experience in a clinic-based setting required.
  • Bachelor of Science in Nursing (BScN)
  • Basic Life Support Certificate (BLS)
  • Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
  • Advanced - Time management skills
  • Advanced - Organizational skills
  • Intermediate - Leadership skills
  • Advanced - Computer skills
  • Demonstrates empathy, trust and respect in the provision of care
  • Knowledge in chronic disease prevention and management
  • Competent critical thinking/judgement/autonomous decision making capabilities
  • Knowledge of the impact of social/economic/cultural issues on chronic disease
  • Demonstrates effective verbal and written communication skills.
  • Demonstrated understanding of the prevention and management of chronic diseases
  • Ability to use health promotion and prevention strategies for program planning
  • Makes independent clinical judgment based on evidence based best practice
  • Knowledge of group dynamics and facilitation skills.
  • Demonstrates ability to accept and implement change
  • Ability to work independently and as a member of a multi-disciplinary team
  • Ability to work under pressure and establish priorities
  • Knowledge in LEAN management system
  • Knowledge and application of quality improvement principles, tools, and outcomes
  • Proven conflict resolution skills
  • Ability to work effectively with individuals who are First Nations or Metis
  • Ability to travel when required

Nice To Haves

  • Experience in teaching adults, facilitation skills, and leading group activities
  • Nursing diploma is acceptable
  • Experience in case management in health care setting is an asset
  • Experience in a community-based setting (Public Health, Home Care) is an asset.
