About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. In this role, described as a Nurse Case Manager, you will guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care. Under the supervision of an RN or MD, you will help ensure patients receive the appropriate level of care, provide education, coaching, and treatment-decision support, and collaborate with a Care Advocate and Social Worker. The position provides home-based patient care and includes patient assessment, care planning, referrals, teaching, supervision of LPNs and HHAs, documentation, communication with the clinical manager and care team, staff education, and assigned on-call responsibilities. You will also participate in clinical development and continuing education programs.

Requirements

Current RN license specific to the state(s) assigned.
Valid driver's license, reliable transportation, and liability insurance.
Current CPR certification.
At least 1 year of clinical experience as a Registered Nurse.

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Make referrals to other disciplines as indicated by the patient's identified needs, or document the rationale for not doing so.
Promote patient health and independence through teaching and appropriate rehabilitative measures, helping patients learn self-care techniques.
Supervise LPNs and HHAs.
Complete documentation timely and accurately.
Regularly communicate patient progress to the clinical manager and care team.
Plan and provide staff education.
Perform assigned on-call responsibilities and provide on-call services to patients and families.
Participate in clinical development and continuing education programs.
Perform other duties as assigned.
