About The Position

Suncrest Hospice is seeking a weekend Registered Nurse (RN) to assist with on-call needs and provide care and services to a group of hospice patients. The RN visits patients where they reside, including private homes, assisted living facilities, and hospitals. Duties include assisting with visits, triage, symptom management, and PRN needs. The schedule is every other weekend, Saturday and Sunday, in 10-hour shifts, with the opportunity to pick up Thursday and Friday shifts.

Suncrest Hospice aims to provide exceptional hospice care by increasing staff-to-patient ratios, clinical visits, and support for patients and their families. The role involves both independent work and collaboration with a patient care team.

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
• At least one year of experience under RN licensure.
• Ability to work effectively in a team environment and collaborate with other care professionals.
• Ability to work independently and communicate by phone, email, and in writing.
• Understanding of hospice rules and regulations.
• Time-management skills to manage visits, documentation, meetings, and urgent needs.
• Ability to learn an electronic medical records system.
• Willingness to drive, with reliable transportation, a valid driver’s license, and auto insurance.
• Availability to work every other weekend on Saturdays and Sundays.

Responsibilities

• Provide care and services to a group of hospice patients.
• Visit patients in private homes, assisted living facilities, and hospitals.
• Assist with patient visits, triage, symptom management, and PRN needs.
• Collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Manage visits, documentation, and meetings; respond to urgent needs by prioritizing and triaging concerns.
• Travel across the service area to visit patients.
