About The Position
Concierge Home Care is seeking a Home Health Registered Nurse (RN) for Assisted Living Facilities (ALF). This role offers the opportunity to take control of your schedule and income, with a focus on 1:1 patient care in assisted living communities. The position is located in Orange Park, FL / Clay County, with territories designed to minimize drive time through clustered scheduling. The role utilizes AI-assisted charting and a user-friendly EMR system to reduce documentation time, allowing nurses to focus more on patient care.
Requirements
- Active, unencumbered Florida Registered Nurse (RN) license
- Valid driver’s license, reliable transportation, and auto insurance
Nice To Haves
- Home health experience preferred, but not required
Responsibilities
- Perform patient assessments and develop individualized care plans
- Provide skilled nursing care and patient education
- Coordinate care with physicians and interdisciplinary teams
- Monitor patient progress and adjust care plans as needed
- Complete documentation using streamlined, AI-assisted tools
