About The Position
Human Touch Home Health is seeking an experienced and compassionate Registered Nurse (RN) to join our growing team serving patients throughout Montgomery County, Maryland. This PRN opportunity offers flexibility, professional independence, and the chance to build meaningful relationships while delivering exceptional patient care. Home health nursing focuses on restoring confidence, supporting recovery, preventing hospitalizations, and helping patients remain comfortable in their homes. As a Home Health Registered Nurse, you will provide personalized, one-on-one care to help patients recover, manage chronic conditions, and maintain independence. You will educate, encourage, and advocate for patients, seeing the direct impact of your care.
Requirements
- Active Maryland Registered Nurse (RN) License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment, Critical Thinking, and Documentation Skills
Nice To Haves
- Home Health Experience Preferred but Not Required
- OASIS Experience Preferred
- Experience with WellSky Home Health is a plus
- Passion for Delivering Exceptional Patient-Centered Care
Responsibilities
- Perform comprehensive nursing assessments and skilled visits
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
- Develop and implement individualized plans of care
- Provide skilled nursing services, including medication management, wound care, disease management, IV therapy as appropriate, and patient education
- Monitor patient progress and communicate changes with physicians and the interdisciplinary care team
- Coordinate care with therapists, social workers, and home health aides
- Complete timely and accurate clinical documentation using WellSky
- Educate patients and caregivers to promote safety, independence, and improved health outcomes
