About The Position

Human Touch Home Health is seeking an experienced and compassionate Registered Nurse (RN) to join our growing team serving patients throughout Prince George's County, Maryland. This PRN opportunity offers flexibility, professional independence, and the chance to build meaningful relationships while delivering exceptional patient care. As a Home Health Registered Nurse, you will provide personalized, one-on-one skilled nursing care to help patients recover from illness, manage chronic conditions, prevent hospitalizations, and maintain independence. The role emphasizes empowering patients and families with knowledge and confidence, focusing on exceptional nursing and meaningful connections rather than a high-volume, assembly-line approach.

Requirements

Active Maryland Registered Nurse (RN) License Required
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment, Critical Thinking, and Documentation Skills

Nice To Haves

Home Health Experience Preferred but Not Required
OASIS Experience Preferred
Experience with WellSky Home Health is a plus
Passion for Delivering Exceptional Patient-Centered Care

Responsibilities

Perform comprehensive nursing assessments and skilled visits
Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
Develop and implement individualized plans of care
Provide skilled nursing services, including medication management, wound care, disease management, IV therapy as appropriate, and patient education
Monitor patient progress and communicate changes with physicians and the interdisciplinary care team
Coordinate care with therapists, social workers, and home health aides
Complete timely and accurate clinical documentation using WellSky
Educate patients and caregivers to promote safety, independence, and improved health outcomes
