About The Position

The Primary Health Care (PHC) Registered Nurse (RN) is an integral member of the primary health care team. This role involves providing and coordinating initial, continuing, and comprehensive nursing services to patients within PHC sites. The PHC RN is responsible for promoting quality health care outcomes by working within established standards for case management practice, ensuring patients/clients receive a high standard of primary health care. This is done in accordance with the Sunrise Health Region mission statement and Integrated Primary Health Services policies and procedures of the facility and department. Additionally, the PHC RN initiates and/or participates in community development initiatives within the site and reports to their respective Primary Health Care manager.

Requirements

  • Two (2) years advanced RN (NP) in applicable area
  • Bachelor of Science in Nursing (BScN)
  • Basic Life Support Level C Certificate (BLS-C)
  • Critical thinking skills
  • Advanced - Organizational skills
  • Public relations skills
  • Basic - Computer skills
  • Advanced - Communication skills
  • Intermediate - Leadership skills
  • Ability to work independently and as a member of a multi-disciplinary team
  • Ability to communicate with clients from diverse cultural backgrounds
  • Demonstrated understanding of the prevention and management of chronic diseases
  • Ability to take initiative
  • Demonstrates ability to accept and implement change
  • Proficient in case management skills and client follow-up
  • Knowledge in applicable health promotion and health prevention initiatives
  • Demonstrated clinical competency
  • Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)

Nice To Haves

  • Nursing experience in Chronic Disease Management
