About The Position

Atrium Health Neurology is located at 315 Medical Park Drive, Suite 202, in Concord, NC. We are a general neurology practice with specialty clinics embedded within our facility. Our Cognitive Program provides comprehensive care for patients diagnosed with Alzheimer’s disease and other forms of dementia. Services include disease management; nurse-led triage and advice via phone and the online patient portal; cognitive testing; and additional care tailored to this patient population. Our clinic participates in the Medicare GUIDE program in North Carolina, which offers specialized dementia care management services to eligible Medicare patients. We provide initial consultations, follow-up visits, and virtual care. Requirements

  • Completion of an accredited or approved program in nursing
  • Registered Nurse license issued by the state in which the teammate practices
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association.
  • Minimum 1-year professional RN experience

Nice To Haves

  • Prior cognitive nursing and Ambulatory care nursing experience preferred
  • Bachelor of Science in Nursing (BSN) preferred

Responsibilities

  • This position includes triage responsibilities; management of care for patients enrolled in the GUIDE program; tracking patients receiving infusion treatments; administering in-depth cognitive testing; and supporting other aspects of care for patients with cognitive disorders.
