About The Position

We are seeking Registered Nurses (RN) to join our Home Health Team in Charlotte, Collier, Lee, and Sarasota Counties. In this role, there is a real impact in the comfort of patients’ homes. As part of our compassionate, high‑performing care team, you’ll provide skilled, patient-centered home health care through assessments, care planning, clinical interventions, patient education, and collaboration with the care team while maintaining accurate documentation and compliance with regulatory standards. This is a Per Diem (PRN) position with variable time and hours can vary from week to week based on staffing needs, patient volume, call-offs, vacations, or organizational requirements. Requirements

• Current and valid Registered Nurse (RN) license in the state of Florida.
• Minimum of 1 year of experience in a hospital or skilled nursing facility.
• Strong clinical skills, excellent communication and interpersonal abilities, and a patient-centered approach to care.
• Strong work ethic, reliability, and a commitment to providing high-quality care.
• Reliable transportation to visit patients in their homes.

Responsibilities

• Conduct comprehensive assessments of patients' physical, mental, and emotional health, including vital signs, medication administration, wound care, and disease management.
• Develop individualized care plans that address patients' specific needs and goals, including medication administration, wound care, and disease management.
• Educate patients and their families on disease management, medication administration, and self-care techniques to promote independence and improve health outcomes.
• Provide direct patient care, including wound care, medication administration, injections, and IV therapy.
• Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and comprehensive care.
• Maintain accurate and timely documentation of patient assessments, interventions, and progress notes.
• Adhere to all relevant regulatory standards, including HIPAA and Medicare regulations.
