About The Position
East Bay Post Acute is seeking a compassionate and experienced Registered Nurse (RN) to join our full-time nursing team. This is an outstanding opportunity for a skilled clinician who is passionate about delivering exceptional resident care and making a meaningful impact in a post-acute setting. As an RN, you will provide direct nursing care, oversee clinical operations, support interdisciplinary care planning, and help ensure the highest standards of resident health and safety.
Requirements
- Current California Registered Nurse (RN) license in good standing
- Current CPR/BLS certification
- Skilled nursing, rehabilitation, or post-acute care experience preferred
- Strong clinical assessment, critical thinking, and leadership skills
- Excellent communication and interpersonal abilities
- Ability to work effectively in a fast-paced healthcare environment
- Commitment to providing compassionate, resident-centered care
Responsibilities
- Provide comprehensive nursing care in accordance with physician orders, care plans, and facility policies
- Perform assessments and monitor resident conditions, identifying and responding to changes in health status
- Administer medications, treatments, and nursing procedures safely and accurately
- Supervise and support LVNs and CNAs in the delivery of resident care
- Coordinate with physicians, therapists, and interdisciplinary team members to ensure optimal resident outcomes
- Complete accurate and timely clinical documentation
- Participate in admissions, transfers, discharges, and care plan meetings
- Ensure compliance with infection control standards, state and federal regulations, and facility policies
- Educate residents and families regarding treatment plans and care needs
