About The Position

We are seeking a dedicated and experienced Registered Nurse to join our team in a Long Term Care/SNF setting. This role requires a strong clinical background, excellent patient care skills, and the ability to adapt to a dynamic healthcare environment. The ideal candidate will have a passion for geriatric care and a commitment to providing high-quality services to our residents.

Requirements

• Active Registered Nurse license
• Current BLS certification
• Minimum of one year of clinical nursing experience
• Recent EPIC electronic medical record experience within the past two years (mandatory)
• Prior travel experience
• Reliable personal transportation

Nice To Haves

• Experience in long-term care or geriatric care environments
• Dementia capable care certification

Responsibilities

• Provide direct patient care to residents in a Long Term Care/SNF facility.
• Administer medications and treatments as prescribed.
• Monitor resident conditions and document observations.
• Collaborate with the healthcare team to develop and implement care plans.
• Educate residents and their families on health conditions and treatment options.
• Maintain accurate and timely electronic health records using EPIC.
• Float between long-term care and memory care units as needed.
• Ensure compliance with all facility policies and procedures and regulatory standards.
