About The Position

This role is for a Registered Nurse in the Inpatient Acute Rehab department at Elmbrook Memorial Hospital in Brookfield, WI. The position is full-time and works the night shift. The nurse will act as the central coordinator for patient care, ensuring seamless management and interdisciplinary collaboration for optimal patient outcomes. This involves implementing, monitoring, and adjusting patient care plans, interpreting and executing physician and nursing orders, maintaining clinical vigilance, documenting findings in the EHR, and communicating significant changes to the healthcare team. Proactive discharge planning, including assessing needs, collaborating with various departments and family, and educating patients for a safe transition home, is also a key responsibility.

Requirements

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.

Responsibilities

• Acts as the central coordinator, ensuring seamless management and interdisciplinary collaboration (physicians, specialists, therapists, social workers, ancillary staff) for optimal patient outcomes.
• Implements, continuously monitors, and dynamically adjusts the prescribed patient care plan based on clinical findings, patient progress, and team input, requiring critical thinking and timely decision-making.
• Precisely interprets, documents, and executes all authorized physician and nursing orders, including medications, tests, treatments, and procedures.
• Maintains constant vigilance, systematically assesses the patient, meticulously documents findings and treatments in the EHR, and immediately communicates all significant changes to the physician and healthcare team.
• Proactively assesses discharge needs from admission, collaborating with the patient, family, social work, case management, and physicians to line up necessary post-care services and teach them everything they need to know for a smooth and safe transition home.
