About The Position

The McCall Centre is a 120-bed chronic care facility situated on the Queensway Health Centre campus. Though a chronic care facility, most patients are transitional in nature for reintegration back into the community. Specialized interprofessional teams work with families to support patients’ plan of care and goals with compassion and excellence.

Requirements

  • Graduate of a recognized School of Nursing
  • Baccalaureate degree required
  • Current certification or eligibility for certification of competence from the College of Nurses of Ontario is required
  • Current BCLS required
  • Recent experience with inpatient medical population required
  • Recent experience with inpatient rehab population and complex care needs population required
  • Recent knowledge of the philosophy and concepts of geriatric rehabilitative nursing practice
  • Demonstrates ability to provide patients and families with educational resources
  • Demonstrates patient and family-centered care
  • Demonstrates knowledge and ability to establish a therapeutic relationship
  • Evidence of on-going professional development
  • Demonstrates critical thinking, problem solving and leadership within a team
  • Must be reliable, flexible, creative, self-directed to work effectively and collaboratively in a high performance interprofessional team
  • Satisfactory attendance and performance required

Responsibilities

  • Responsible and accountable for decisions and actions of the provision and management of nursing care for patients
  • Provides nursing assessment, contributes to the care plan development, provides goal oriented treatment/interventions, patient and family education and support to address medical, cognitive, emotional and self-care needs
  • Contributes to the development of a realistic discharge plan
  • Acts as a coach/mentor/consultant and uses sound critical thinking skills which are key to achieving patient outcomes and facilitating team functions
  • Participates in program planning, continuous quality improvement and evaluation
  • Willingness to take on unit clinical leadership during the shift
