About The Position
Aspire Home Healthcare is seeking passionate and skilled Registered Nurses (RNs) to join their team and make a difference every day by delivering quality, one-on-one care that helps patients live comfortably and safely in their homes. This is a part-time or per diem opportunity with competitive per-visit pay, flexible scheduling, and mileage reimbursement. Full-time employees also receive comprehensive benefits.
Requirements
- Active RN license in California
- Current physical exam (within last 6 months)
- Current TB test (within last 12 months) and CPR certification
- Valid driver’s license
- Must be able to attend orientation at our Yuba City office
Nice To Haves
- Experience with OASIS and WellSky (Kinnser) strongly preferred
- Med-surg and wound care experience a plus
Responsibilities
- Obtain medical history and assess patient condition and needs
- Conduct physical assessments, including vitals and mobility evaluations
- Educate patients and families on care plans and home health services
- Deliver skilled nursing care while maintaining HIPAA compliance
- Document and submit accurate visit records promptly
- Develop and update individualized care plans as needed
- Coordinate with LPNs, and Home Health Aides
- Facilitate referrals for additional services such as therapy or medical social work
- Evaluate the effectiveness of nursing services and adjust as appropriate
