About The Position

Aspire Home Healthcare is seeking a part-time Medical Social Worker (MSW) for the Yuba City area. Established 26 years ago, Aspire focuses on putting patients first by providing around-the-clock care and support. This role involves providing evaluation and treatment to patients facing social and personal adjustment issues that impact their independent living or are a reaction to their medical diagnosis. The MSW will be responsible for developing treatment plans and offering individual, family, or group therapy.

Requirements

  • Master’s degree in medical social work or social work license in CA
  • 2 years of social work experience in a medical setting, preferably in home health
  • Current CPR Certification
  • Valid driver’s license and car insurance
  • Ability to attend a 2-3 day orientation at our Yuba City office

Responsibilities

  • Provides psychosocial evaluation, diagnosis, treatment, and referral for patients and/or families, including patient and/or family goals.
  • Interprets evaluation results and recommends treatment to the patient/family referral source.
  • Provides services such as community referral, individual therapy, family therapy, group therapy, consultation, or education.
  • Aids the health care team in understanding psychosocial factors related to the patient’s illness, treatment, and recovery.
  • Plans and leads individual and family therapy, patient and family groups, and family conferences.
  • Provides written reports for patient medical records (evaluation, progress notes, discharge summary) with copies to referral sources, patients, and families when indicated.
  • Maintains accurate and up-to-date client records in the client chart.
  • Consults with physicians, families, and referral sources regarding client progress and therapy programs.
  • Maintains all departmental records in accordance with company policy, state, and federal regulations.
  • Maintains confidentiality of client situations, records, and services provided according to HIPAA regulations.
  • Discusses patient problems and resolutions, changes in patient needs, and the need for other health team involvement with the Director of Clinical Services.
  • Coordinates total patient care.
