About The Position

We are seeking to hire a Registered Nurse to be part of our Care Coordination/Case Management Team. St. Louis County offers competitive pay and a robust benefits package. Duties of this role include: Coordinating care and services to clients age 65+ that are enrolled with Blue Cross and Blue Shield managed care plans. Completing comprehensive health risk assessments at least annually for health and social needs. Developing, reviewing, and maintaining individualized, comprehensive care plan for clients. Serving as the client's advocate. This full-time position could be based in Duluth, Hibbing, or Virginia, MN based on the preference of the selected candidate. The salary range reflected above is the normal hiring range for 2026. The full salary range, including longevity pay, is $31.21 - $44.23 per hour. These positions are part of the Civil Service Basic Unit Employees Collective Bargaining Agreement represented by Council 5 of AFSCME. St. Louis County does not participate in E-Verify. All employees must be continuously authorized to work in the United States, for St. Louis County, on a full-time basis. St. Louis County does not provide sponsorship for employment visas.

Requirements

Graduation from an accredited college or university with an Associate’s degree in nursing.
Possession of a current Minnesota Registered Nurse license.
Possession of a valid driver’s license.
Two (2) years full-time paid verifiable experience in a home or community-based setting.
Possession of MnCHOICES Certified Assessors certification within two (2) months of employment.

Responsibilities

Coordinating care and services to clients age 65+ that are enrolled with Blue Cross and Blue Shield managed care plans.
Completing comprehensive health risk assessments at least annually for health and social needs.
Developing, reviewing, and maintaining individualized, comprehensive care plan for clients.
Serving as the client's advocate.
Providing comprehensive assessments, educational counseling, and providing case management and care coordination depending on the program area.
