About The Position

We are currently pursuing a passionate and caring part-time night shift, Registered Nurse to join our Pole Creek Estates Long-Term Care team. At Pole Creek Estates, our residents are our number one priority. Requirements

  • Graduate from an accredited Registered Nurse Nursing program.
  • License as a Registered Nurse in the State of Nebraska.
  • Basic Life Support (BLS) certification.

Responsibilities

  • Provide prescribed medical treatment and personal care services to residents in Long-Term Care.
  • Assess, plan, intervene, and evaluate resident care to achieve prioritized resident outcomes utilizing the nursing process for the performance of resident care.
  • Perform complete admission assessment in accordance with Long-Term Care’s policy and procedures, including full body system analysis and home med reconciliation process for the resident in the appropriate care setting.
  • Collect, evaluate, analyze, document, and prioritize pertinent resident data using the full range of resident needs: physical, psychosocial, spiritual, educational, and discharge.
  • Communicate effectively between resident, resident’s family, physicians, and other staff members of the healthcare team. Coordinate and evaluate the resident’s care to ensure continuity.
  • Perform and maintain documentation to accurately reflect the plan of care.
  • Communicates and collaborates effectively with other health team members regarding resident’s condition, nursing plan of care, resident satisfaction needs, and recommendations for meeting identified outcomes.
  • Promote and ensure patient safety in performance of all responsibilities.
  • Develop a plan of resident care reflecting significant nursing diagnosis and interventions to achieve desired resident outcomes, involving the patient/family and healthcare team in this process.
  • Provide education as indicated after assessing the learning needs of the patient, family, staff members, and students.
  • Supervise other healthcare team members according to agency, state, and federal requirements.
  • Assists in the development of quality indicators, thresholds, study methods, and data collection as assigned.
  • Develop and implement social work/discharge planning services, human resources, and financial goals and objectives consistent with Long-Term Care’s goals and objectives.
  • Provide information to resident and families about types and costs of services available to enable them to make informed decisions.
  • Other duties as assigned by management.
