About The Position

Newport Hospice is seeking highly skilled, compassionate, and detail-oriented Registered Nurses to join our team in a Per Diem capacity. Requirements

• Active RN License in the state of California.
• Deep understanding of hospice regulations, LCD guidelines, and nursing requirements.
• Exceptional clinical assessment skills.
• Ability to manage complex symptoms at the end of life.
• Elite verbal and written communication skills.
• Proficiency in EMR systems.
• Flexibility to cover various locations within our service area.

Responsibilities

• Perform comprehensive assessments, including full head-to-toe physical assessments.
• Conduct medication management, including reconciliation, pharmacy coordination, and order review.
• Review, create, and update the patient’s Plan of Care (POC).
• Ensure all documentation meets LCD guidelines and California state hospice regulations.
• Facilitate the signing of consents and provide a thorough Explanation of Benefits (EOB) for SOC visits.
• Communicate directly with MDs for orders, symptom management, and coordination with the Interdisciplinary Group (IDG).
• Submit charting in a timely and precise manner, prioritizing 'documenting as you go'.
