About The Position

Trillium Health Partners is one of Canada’s largest community-based teaching hospitals, serving the growing and diverse populations of Mississauga, West Toronto, and surrounding communities. The Reactivation Care Centre (RCC) is Trillium Health Partners (THP) fourth site, located at 200 Church Street in York. At the RCC, the goal is to reintegrate individuals who no longer need acute medical care back into the community. The specialized team can support patients’ care plan goals while providing more social engagement and recreational therapies.

Requirements

• Current registration with the College of Nurses of Ontario
• Current BCLS certificate
• Ability to perform phlebotomies, tracheostomy care and IV starts
• Experienced in complex continuing care, gerontology and long term care
• Effective interpersonal, organizational and communication skills
• Critical thinking and leadership skills
• Reliable, flexible, creative, self-directed professional able to work effectively and collaboratively in a high performance interdisciplinary team

Responsibilities

• Responsible and accountable for decisions and actions in the provision and management of nursing care for patients.
• Provides nursing assessment, contributes to care plan development, goal oriented treatment interventions, patient and family education and support to address medical, cognitive, emotional and self-care needs.
• Contributes to the development of a realistic discharge plan.
• Performs as a coach/mentor/consultant and uses sound critical thinking skills, which are key to achieving patient outcomes and facilitating team function.
• Participates in program planning, continuous quality improvement and evaluation.
