About The Position

We are seeking a Registered Nurse (RN) to support patient‑centered care delivery within Capital Health Plan's Nursing department. This role provides direct patient care, health promotion, disease prevention, and patient education while working closely with providers to ensure care is coordinated, team‑based, and aligned with community needs. The Registered Nurse identifies patient care needs through in‑person, telephone, or electronic interactions, documents care in the Electronic Health Record (EHR), develops and implements plans of care, and performs patient care services in accordance with the Nurse Practice Act and Capital Health Plan's policies and procedures. This role also supports proactive care by identifying care gaps and facilitating appropriate follow‑up.

Requirements

  • Associate's (ASN) or Bachelor's degree (BSN) in Nursing from an accredited college or university
  • Active Registered Nurse (RN) license in the State of Florida
  • Basic Life Support (BLS) certification
  • Ability to assess patient needs and provide care through in-person, phone, or electronic modalities
  • Ability to document patient information accurately in an Electronic Health Record
  • Strong knowledge of infection control practices, medication administration, and patient safety standards
  • Ability to communicate effectively with patients, providers, and interdisciplinary care teams

Nice To Haves

  • Minimum of two years of related nursing experience
  • Experience working in outpatient, primary care, or ambulatory settings
  • Experience identifying and addressing gaps in care using clinical tools or registries
  • Experience developing and implementing care plans independently and collaboratively
  • Strong proficiency with clinical systems and healthcare technology
  • Knowledge of CHP policies, workflows, or population health concepts
