Requirements

  • Bachelor's Degree in Social Work, Human Services, Public Health, Gerontology, and/or a related field or equivalent experience required.
  • Proficiency with Microsoft Office applications such as Word, Excel, Outlook, and Teams is required.
  • Candidates should be comfortable working with client database systems and learning new software tools as needed.
  • Previous experience in the delivery of evidenced-based programs is highly preferred, as well as excellent communication, client services, and data collection skills.
  • The appropriate person will be highly organized and detail-oriented.
  • This position requires personal and professional accountability; self-management and initiative; dedication to quality improvement; professional integrity; research ethics, and cultural competency.
  • Must demonstrate accuracy, efficiency and superior organizational skills.
  • Must demonstrate effectiveness in the following areas: task and time management; teamwork; and communication with teams, managers, clients and participants, and company consultants.
  • Works with standard office equipment including computer work station, printers, copiers, scanners and fax machine.
  • Works with word processing software, spreadsheet applications, and presentation software.
  • Communicates with clients and vendors using the telephone, e-mail, the Internet, and fax.
  • Requires clear speaking ability and the ability to use a telephone for extended periods.
  • Requires ability to view a computer screen (near acuity).
  • Requires fingering agility for keyboarding.
  • The ability to sit, stand, and engage in extended periods of screen time or communication by phone is necessary to perform the essential duties of the role.

Nice To Haves

  • 2 years case management experience and training or 3 years Promotora experience, preferred.
  • Spanish language proficiency and multicultural experience strongly preferred.

Responsibilities

  • Provides specialized Case Management, based on levels of acuity, to a caseload of assigned clients 50 years old or older.
  • Maintains ongoing, proactive contact to assess and monitor client’s physical, cognitive and psychosocial status and needs through personal visits, telephone and written contacts.
  • Conducts comprehensive intake and needs assessments to identify client needs, including but not limited to: Access to healthcare and the client’s ability to self-manage and actively participate in healthcare – both HIV and age-specific.
  • Adherence medication and medical treatment
  • Short term and long term housing needs
  • Income stability including employment and/or retirement needs, as well as eligibility for public benefits (SSI, SSDI, Medicaid, Medicare, etc.)
  • Mental health and substance abuse issues using program specified assessment tools.
  • Food and nutrition
  • Risk reduction counseling
  • Collaborate with clients or their designated/appointed caregivers, use geriatric screening results to develop strategies and goals in an Individual Wellness Plans, to meet identified needs and improve health outcomes.
  • Maintains timely Individual Wellness Plans according to program policies.
  • Assist with processing of emergency financial requests.
  • Maintains quality and timely session documentation on all client interactions, as well as referral to and follow-through Wellness Plan goals and aging referrals
  • Collects service eligibility documentation in a timely manner according to program policies.
  • Assess caseloads on a regular basis to determine potential changes in client acuity.
  • Inactivates cases based on services usage according to program policies and in collaboration with Community Health Innovation team members
  • Collaborates and coordinates services with other HIV/AIDS and aging service providers and community agencies.
  • Identifies internal and external resources to meet client needs, and refers clients accordingly.
  • Complete training to use of geriatric screening and assessment tools, integrating tools into workflow processes and procedures.
  • Coordinates/Attends Case Conferences with CHN Housing team, CHN Behavioral Health team, CHN Prevention team, CHN Healthy Aging Programs and other community providers as needed.
  • Must be able to work in a diverse setting with diverse populations, including persons living with HIV, the LGBTQ+ community, persons of various ethnic backgrounds, disenfranchised communities.
  • Participation in training related to HIV, substance use, harm reduction, trauma informed care and related issues.
  • Cultural Competency/Sensitivity.
  • Other duties may be assigned.
