About The Position

Partner with families of children 0-5 as they tackle life's stressors by home visiting through the Healthy Families America model. Provide direct services to individuals and families referred to the Healthy Families program in accordance with Healthy Families America’s Best Practice Standards. This role requires maintaining a reliable level of attendance and scheduling based on participant needs. Responsibilities include interviewing and assessing family history and strengths, planning and preparing for weekly visits, and recording visits and activities in the EHR system within 48 hours. The position also involves attending staff meetings, reviewing case updates with a clinical supervisor, and completing 40 hours of staff development annually, including specific credits in diversity, Bloodborne Pathogen, wellness/self-care, CMS training, and abuse/neglect reporting, as well as continuing education relevant to the role.

Requirements

  • Bachelor’s degree in related field with minimum of three years’ relevant experience, preferred.
  • Working knowledge of child development, behavioral management techniques, family system relationships, communication skills, ability to teach self-sufficiency skills, community resources, recreational activities, ability to teach stress management techniques, interviewing skills and assessment skills.
  • Able to utilize computer programs specific to the position (Microsoft Office Suite, DAISEY, INSYNC, and others as necessary).

Responsibilities

  • Provide direct services to individuals and families referred to the Healthy Families program; in accordance to Healthy Families America’s Best Practice Standards
  • Maintain a reliable level of attendance necessary to perform the job; scheduling to be based on what best meets the needs of our participants and Family & Children’s Center.
  • Interview and assess family history and strengths of those families newly referred to Healthy Families Program.
  • Adequately plan and prepare for each weekly visit with referred and enrolled families
  • Record visits and activities, within 48 hours, in the EHR system.
  • Attend and participate in staff meetings.
  • Review case updates with clinical supervisor.
  • Complete 40 hours per year staff development (20 for part/time) including a diversity credit, Blood borne Pathogen credit, wellness/self-care credit, CMS (billing fraud) training, abuse/neglect indicators & reporting requirements and continuing education in topics directly related to this position.
