About The Position

We have a growing emphasis on serving Veterans and those who have served our country, providing personalized care and support tailored to each patient's individual needs. UPLIFT HOMECARE, LLC is seeking a skilled and compassionate Registered Nurse (RN) to join our growing home health team. The ideal candidate is an experienced clinician who is comfortable providing independent, patient-centered care in the home setting and collaborating with patients, families, physicians, therapists, and other members of the healthcare team. As an RN with UPLIFT HOMECARE, you will play an important role in assessing patient needs, developing and implementing individualized plans of care, coordinating services, and ensuring the delivery of safe, high-quality clinical care.

Requirements

  • Current, unrestricted Florida RN license – Required
  • Home health experience – Preferred/Required based on position
  • OASIS experience strongly preferred
  • Experience with EMR/EHR systems; Axxess experience is a plus
  • AHCA Level II Background Screening
  • Current CPR certification
  • Valid driver's license
  • Current auto insurance
  • Reliable transportation
  • Strong clinical assessment and critical-thinking skills
  • Excellent written and verbal communication skills
  • Ability to work independently while maintaining strong communication with the clinical team

Responsibilities

  • Conduct initial, comprehensive, and ongoing assessments of patients' clinical needs.
  • Participate in interdisciplinary assessment and care planning.
  • Develop, implement, and evaluate individualized plans of care in collaboration with the patient, representative, and caregivers.
  • Perform skilled nursing assessments and interventions in the home setting.
  • Administer medications, injections, treatments, and other nursing interventions in accordance with the plan of care and applicable standards.
  • Monitor vital signs, assess patient conditions, and evaluate responses to treatment.
  • Identify changes in patient condition and communicate appropriately with physicians and other members of the healthcare team.
  • Provide patient and caregiver education regarding medications, treatments, disease management, safety, and care needs.
  • Coordinate care with physicians, therapists, case managers, caregivers, and other interdisciplinary team members.
  • Complete accurate, timely, and thorough documentation in the agency's EMR/EHR system.
  • Maintain compliance with HIPAA, agency policies, and applicable state and federal regulations.
  • Participate in Quality Assessment and Performance Improvement (QAPI) activities.
  • Participate in required in-services, training, and continuing education.
  • Utilize sound clinical judgment to effectively manage individual patient needs.
  • Provide compassionate, professional care that promotes patient dignity, independence, and quality of life.
