Requirements

  • Graduate of an accredited Registered Nurse (RN) program.
  • Minimum of one (1) year of experience working with medically complex individuals.
  • Current and valid State RN License in good standing.
  • Current CPR / First Aid certification.
  • Valid driver’s license, active car registration, and auto insurance (required for providing participant transportation).

Responsibilities

  • Conduct thorough medical assessments for incoming individuals, partnering closely with the Intake team to ensure appropriate community placement.
  • Collaborate with physicians to develop, implement, and routinely update individualized service and nursing care plans that prioritize long-term health and independence.
  • Generate robust healthcare protocols for specific medical conditions outlined in individual health and care plans.
  • Manage and administer medications in strict accordance with clinical guidelines, ensuring meticulous documentation and compliant record reviews.
  • Oversee the clinical care of high-risk medical conditions and coordinate private duty nursing services as needed.
  • Maintain seamless communication with primary care physicians and specialists, accompanying individuals on medical visits when required.
  • Design tailored medical training tools and deliver clinical skills training to clients, staff, mentors, respite providers, and biological families.
  • Present vital medical skills components during Pre-service Orientation for new team members.
  • Actively contribute to Individual Health Plan (IHP) teams, Human Rights Committees, and Health & Safety administrative meetings.
  • Participate in an on-call system and assist with essential transitional or household coordination—such as safe transportation to appointments—to guarantee continuity of care.
