Requirements

Graduate of an accredited Registered Nurse (RN) program.
Minimum of one (1) year of experience working with medically complex individuals.
Current and valid State RN License in good standing.
Current CPR / First Aid certification.
Valid driver’s license, active car registration, and auto insurance (required for providing participant transportation).

Responsibilities

Conduct thorough medical assessments for incoming individuals, partnering closely with the Intake team to ensure appropriate community placement.
Collaborate with physicians to develop, implement, and routinely update individualized service and nursing care plans that prioritize long-term health and independence.
Generate robust healthcare protocols for specific medical conditions outlined in individual health and care plans.
Manage and administer medications in strict accordance with clinical guidelines, ensuring meticulous documentation and compliant record reviews.
Oversee the clinical care of high-risk medical conditions and coordinate private duty nursing services as needed.
Maintain seamless communication with primary care physicians and specialists, accompanying individuals on medical visits when required.
Design tailored medical training tools and deliver clinical skills training to clients, staff, mentors, respite providers, and biological families.
Present vital medical skills components during Pre-service Orientation for new team members.
Actively contribute to Individual Health Plan (IHP) teams, Human Rights Committees, and Health & Safety administrative meetings.
Participate in an on-call system and assist with essential transitional or household coordination—such as safe transportation to appointments—to guarantee continuity of care.
