Requirements

Bachelor’s Degree in Nursing
Current New York State RN license
BCLS Certification from the American Heart Association

Nice To Haves

At least five years of acute care RN experience in Dialysis
At least two years of chronic care RN experience in Dialysis
Master’s Degree in Nursing
BONENT or NNCB certification

Responsibilities

Educate patients and caregivers on dialysis techniques
Provide direct patient care in the chronic dialysis unit as needed
Conduct assessments to ensure patient readiness and safety for home therapy
Monitor patient progress and troubleshoot issues both remotely and in person
Collaborate with interdisciplinary teams to develop and adjust individualized care plans
Offer emotional support and guidance to patients transitioning to or from home therapy
Maintain accurate documentation and ensure compliance with regulatory standards
Participate in quality improvement initiatives and staff development
