About The Position

Suncrest Hospice is seeking a weekend Registered Nurse to assist with on-call needs. The role works every other weekend on Saturday and Sunday in 10-hour shifts, with an opportunity to pick up shifts on Thursday and Friday. The RN provides care and services to a group of hospice patients, visiting them where they reside, including private homes, assisted living facilities, and hospitals. Duties include assisting with visits, triage, symptom management, and as-needed care. Suncrest Hospice emphasizes exceptional patient and family care, increased clinical visits, and lower clinician caseloads. The organization is CHAP-certified.

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
• At least one year of experience under RN licensure.
• Ability to work every other weekend on Saturdays and Sundays.
• Ability to work effectively within a team and collaborate with other care professionals.
• Ability to work independently.
• Time-management skills to coordinate visits, documentation, meetings, and urgent needs.
• Ability to learn an electronic medical records system.
• Willingness to drive across the service area with reliable transportation, a valid driver's license, and auto insurance.
• Understanding of hospice rules and regulations.

Responsibilities

• Provide care and services to a group of hospice patients.
• Visit patients in private homes, assisted living facilities, and hospitals.
• Assist with patient visits, triage, symptom management, and as-needed care.
• Manage visits, documentation, and meetings; respond to urgent needs by prioritizing and triaging multiple concerns.
• Collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Communicate by phone, email, and in writing, and meet with healthcare professionals.
• Work independently while coordinating with the patient care team.
