Registered Nurse - In-Home Care Assessments & Care Planning
About The Position
This Registered Nurse position focuses on in-home assessments and care planning without direct or physical patient care. The RN visits clients at home, speaks with clients and families, reviews health histories, and uses verbal and observational assessments to understand health backgrounds, functional needs, daily routines, and personal goals. The RN develops and updates practical, individualized care plans for non-medical in-home caregivers to follow, and serves as a clinical resource to the care team. The work centers on conversation, observation, documentation, and relationship-based support rather than clinical tasks or procedures. The role involves scheduled visits to clients’ homes and collaboration with the office team to support care delivery. It is suited to nurses seeking to move away from physically demanding clinical environments and bedside responsibilities while continuing to apply nursing judgment through assessment, care coordination, planning, and education.
Requirements
- Active Registered Nurse (RN) license.
- Ability to complete thorough verbal and observational assessments.
- Strong documentation and care-planning skills.
- Comfortable working independently in the community.
- Professional, compassionate communication style with clients, families, and caregivers.
Responsibilities
- Travel to clients’ homes to complete scheduled, non-clinical nursing assessments.
- Conduct verbal and observational assessments with clients and family members.
- Review medical history, diagnoses, medications, functional abilities, and daily routines.
- Identify risks, support needs, and appropriate non-medical services.
- Develop and update individualized care plans for in-home caregivers to follow.
- Serve as a clinical resource to the care team regarding the client’s plan of care.
- Maintain accurate and timely documentation of assessments and care plans.
- Collaborate with the office team to support care delivery.
