The Registered Nurse provides general medical care and treatment to patients under the direction of medical providers. This position is per diem/as needed. The main clinic will be Monfort Family Clinic, but the nurse must be able to float between other clinics. With a Quality, Customer First, and Compassionate approach, The Registered Nurse will provide disease case management for complex patients with chronic conditions and patients undergoing diagnosis and treatment for TB Infection (LTBI). They will support and back up the Obstetrics RN occasionally by providing high-quality care to expectant mothers, stepping in as needed to assist with patient assessments, non-stress testing, case management, and ensuring a safe and nurturing environment for patients. The role involves conducting medication reconciliation for annual wellness visits, focusing on preventive care and health promotion, and monitoring patients according to guidelines set by CDC, CCGC, State and Sunrise Community Health. The nurse will assist clinicians in prepping appropriate patients with history, treatments, exams, diagnostic procedures, and medication administration, monitoring patients for reactions or side effects. They will assist in ordering and maintaining medications, maintain complete and accurate patient records, record patient medical information and vital signs, and advise patients on health maintenance and preventative care. The role includes monitoring, recording, and reporting symptoms or changes in patient’s conditions, consulting and coordinating with healthcare team members to assess, plan, implement, or evaluate patient care plans, and modifying treatment plans where indicated. The nurse will support Medical Assistants and medical staff with nursing triage questions, perform case management review as assigned (DM, OB, Cardiovascular Disease, Asthma, LTBI, etc.), and work closely with Director of Nursing, Chief Clinical Officer, Medical Directors, and site management to ensure efficient and consistent compliance with clinical and operational policies and procedures. They will ensure patient needs are appropriately addressed through scheduling and triage, support the team to meet patient needs, and participate in collaborative activities as designated by CCO and CPSO. The nurse will follow and initiate policy for standing orders and provide quarterly training to staff on current health care issues or medical procedures. This role emphasizes Team Based Care, Clinical Care Collaboration, and Pre-Visit Team Based Planning, working in collaboration and continuous partnership with providers, clinical staff, and community resources to promote access to appropriate care and enhance patient health. The nurse will review the daily schedule and chart by care team, ensure documented instructions are written on the schedule, typed into the EHR appointment note, or in the EHR as a flag or clinical list update. The nurse will accept other clinical or administrative nursing assignments from CCO or DON.
