Optum is seeking a Homecare Nurse for their Women's and Children's Health - Homecare division. This role involves providing skilled nursing care in a home setting, including assessment, planning, documentation, and patient/family education. The nurse will travel to patients' homes, with visits typically scheduled within 24-48 hours of referral. The position requires upholding company values, adhering to privacy guidelines and regulations, and providing services according to Optum policy and the prescriber's plan of treatment. The nurse will individualize patient assessments, ensure interventions respect patient choices, and educate patients and families systematically. They will initiate and modify care plans based on assessments and physician orders, coordinate care with other healthcare professionals, and communicate patient status to physicians and other relevant parties. Clinical documentation must be completed accurately and in a timely manner. The role may involve providing clinical support to physicians/hospitals, participating in holiday and weekend duties, maintaining CPR certification, and complying with annual health screenings and drug testing. Routine travel is required, and the nurse may be asked to participate in training or act as a preceptor. Additional duties may be assigned, including home health administrator responsibilities and special project management.
