About The Position

This role facilitates and coordinates all transplant-specific phases of care and processes for transplant recipients. The Transplant Coordinator maintains documentation required for certified transplant centers and collaborates with a multidisciplinary team to provide individualized patient care. This position also participates in quality improvement efforts aligned with the program, department, and hospital's strategic plan. Requirements

Bachelor's degree in Nursing.
Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.
Certified Clinical Transplant Coordinator or Certified Clinical Transplant Nurse (CCTC or CCTN) required or able to be completed within 2 years of hire.

Nice To Haves

Master's degree in Nursing preferred.
Two (2) years RN experience in a transplant program preferred.
In lieu of the BSN requirement, a nursing diploma or an Associate Degree in Nursing (ADN) may be considered if the applicant has previous transplant nursing experience and signs a Memorandum of Understanding agreeing to enroll in an accredited BSN or MSN program within one year and obtain a BSN or MSN degree within four (4) years.

Responsibilities

Advocates for the patient and family, providing resources and coordinating care to help them navigate the healthcare system.
Empowers patients and families to actively participate in care decisions and become independent in managing care at home.
Maintains thorough, accurate, and timely electronic documentation of all patient encounters to ensure safe transitions between transplant/donation phases.
Communicates effectively with patients, families, multidisciplinary team members, referring providers, and consultants regarding patient status and care plans.
Maintains and ensures communication with patients, referring physicians, payors, and other healthcare providers throughout the transplant/donation process.
Collaborates with nursing staff, social work, and other healthcare providers to establish and implement patient care plans according to current standards.
Maintains professional knowledge and clinical ability to provide evaluation and treatment for complex patients, including those in abdominal transplantation.
Interprets clinical data and actively participates in medical decision-making care teams.
Ensures comprehensive healthcare needs (medical, psychosocial, economic) are continually monitored and addressed.
Monitors and updates the patient's status on the national registry waiting list.
Stays current on clinical policy, guidelines, and regulatory changes.
Maintains current knowledge of transplant center outcomes and performance standards.
Provides individualized patient/family education throughout all phases of care and assesses its effectiveness.
Plans, conducts, and evaluates educational materials and activities for patients and families.
Educates candidates and families about the evaluation process, regulations, allocation, pre-transplant listing, transplant process, confidentiality, post-operative care, infection/rejection symptoms, immunizations, financial issues, and responsibilities.
Assesses candidate and family educational deficits regarding the transplant process and reinforces education and follow-up requirements.
Participates in, contributes to, and promotes continuing education on transplantation and patient care.
Participates in improvement initiatives and projects within the institution.
Serves as a resource to internal and external organizations for transplant-specific education.
Maintains current knowledge of and complies with institutional policies and procedures.
Demonstrates knowledge of current regulatory oversight requirements for transplant centers and actively participates in survey readiness and site visits.
Facilitates/coordinates the evaluation and listing of potential transplant candidates.
Identifies requirements and oversees the candidate evaluation process.
Interprets serological results and their implications for transplant.
Gathers and communicates serological results and blood type according to regulatory requirements.
Identifies possible co-morbidities and contraindications for potential transplant candidates.
Functions as a candidate advocate, applying knowledge of growth, development, education, and cultural background.
Collects and evaluates medical, psychosocial, and financial data for transplant team review.
Communicates candidate status and evaluation outcomes to referring physicians, care providers, payors, and transplant team members.
Provides data to regulatory bodies for listing statistics.
Monitors and coordinates the care of candidates awaiting transplantation.
Ensures monitoring per transplant center specific standards.
Identifies and responds to psychosocial and economic needs of transplant candidates during the pre-transplant period.
Maintains and updates candidate listing status per regulatory regulations.
Identifies the need for candidate re-evaluation and initiates the process.
Participates in the transplant/donation process according to specific standards.
Shares pertinent information about the intended recipient, including ABO compatibility, with the transplant team.
Facilitates admission or transfer of patients to the transplant facility.
Notifies the transplant team of pending medical and/or surgical procedures.
Removes transplant recipients from the list according to regulatory guidelines.
Provides outcome specifics to regulatory bodies for donor family follow-up.
Demonstrates understanding of post-transplant management, including medications, complications, interventions, and discharge planning.
Coordinates the care of transplant recipients in the outpatient setting for optimal rehabilitation.
Identifies complications and considers interventions with the transplant team.
Coordinates discharge planning for continuity of care.
Assesses recipient compliance with medication and medical regimens and collaborates with the transplant team to achieve and maintain compliance.
Collects and reviews data pertinent to recipient/donor health and organ function with the transplant physician.
Communicates adjustments in patient care plans and drug therapy as ordered by medical providers.
Arranges for consults, diagnostic testing, and hospitalization when indicated.
Coordinates emergent evaluation of transplant recipients when indicated.
Assists in reporting required data to regulatory bodies.
Performs other duties as assigned.
