About The Position

We are hiring a full-time Transitional Care Management RN. The Role You will assist patients recently discharged from acute care. You will gather clinical data and help prevent hospital readmissions.

Requirements

• Active Ohio RN license.
• Strong critical thinking and time management.
• Clear, professional, and empathetic phone skills.

Nice To Haves

• BSN degree is preferred.

Responsibilities

• Conduct post-acute phone assessments.
• Identify barriers to patient recovery.
• Review discharge instructions, medications, and referrals.
• Facilitate and schedule face-to-face follow-up visits.
• Log all patient calls accurately in the EMR system.
• Maintain patient privacy and strictly follow regulatory standards (OSHA, ODH).
