Responsibilities
- Serve as a trusted resource for nurses, physicians, caregivers, and loved ones.
- Help maintain comfort, dignity, and continuity of care.
- Work independently in a field-based or home-care setting.
- Support colleagues.
- Provide calm reassurance.
- Uphold the highest standards of patient-centered care.
- Perform Service Intensity Add-on (SIA) visits for Hospice patients during the last seven days of a patient's life, providing more intensive and focused care during the final stages of life.
- Drive to patient's location for visits.
