Registered Nurse - Stroke Program Data Coordinator

Responsible for concurrent chart review and data abstraction for stroke patient records, reporting data, and providing feedback to Neuro Program Coordinators, maintain discharge call back program, and maintaining EMMI program. Requirements
- Current Illinois Registered Nurse
- 5 years RN experience in an acute setting.
- Intermittent hand manipulation required
- Intermittent lifting and carrying of 20 lbs.

Responsibilities
- Maintains post discharge call back program including EMMI for all stroke patients
- Primarily responsible for concurrent data abstraction of stroke patient records.
- Provide feedback to nursing staff and Neuro Program Coordinators based off of data abstracted to help improve Stroke Program
- Presents stroke data bimonthly at Stroke Committee Meetings
- Report stroke data at CPIC meetings
