About The Position

This position serves as the RN-SPECIAL NEEDS nursing administrator responsible for planning, managing, coordinating, directing, and evaluating all aspects of the installation serving Nellis and Creech EFMP-M program operations and administrative functions in accordance with DoD and Air Force policy. The role involves interpreting and ensuring compliance with various DoD and AF instructions and local policies, acting as the technical authority for EFMP medical enrollment and eligibility processes. The coordinator provides guidance and training to various stakeholders, including Family Member Relocation Clearance Coordinators, administrative staff, TRICARE representatives, service members, and families. Key responsibilities include guiding patients and families through the EFMP process, coordinating care, acting as a liaison between services and agencies, and advocating for patients. The role also involves establishing procedures to identify families with special needs, conducting assessments, assisting families in obtaining information on required services, and ensuring access to necessary medical, behavioral, and educational services. This includes providing follow-up assistance, monitoring cases for eligibility, performing case reviews, and documenting care in medical records and databases. Collaboration with various agencies and maintaining up-to-date databases on EFMP enrollees are crucial. The position also requires developing marketing strategies, preparing and delivering briefings on services, and maintaining good interpersonal relationships with colleagues and families. The role emphasizes excellent oral communication skills, collaboration with a diverse group of people, and adherence to safe work environment practices.

Requirements

  • Graduate from a baccalaureate degree (BSN) program in nursing accredited by a national nursing accrediting agency recognized by the US Department of Education.
  • Maintain current certification in either the American Heart Association or American Red Cross Basic Life Support (BLS) for Healthcare Providers without lapses.
  • Minimum of 4 years nursing experience in a direct patient care clinical setting as a Registered Nurse.
  • Last 2 years full-time experience includes care coordination, discharge planning or case management in a medical setting for patients ranging in age 0-95 years.
  • Possesses broad and complex medical knowledge required for care coordination and management.
  • Ability to provide clinical and operational knowledge on hospital and nursing standards, regulations, policies, and excellent communication and interpersonal skills.
  • Knowledge and understanding of developmental growth dynamics of human behavior, family and other social systems; and impact of illness and disability on social functioning.
  • Knowledge of medical facility organization, mission, purpose, philosophy, and objectives as well as those of the ANA and TJC to provide professional nursing consultation to others in the MTF.
  • Skill in oral and written communication to articulate medical requirements to professional and administrative staff and to present briefings to providers and beneficiaries.
  • Ability to analyze and evaluate treatment plans and their relationship/appropriateness to diagnoses and to recognize questionable treatment plans.
  • Familiarity with a variety of computer applications/software to input and extract data from established databases.
  • Permanent, full, and unrestricted Registered Nursing license to practice nursing in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

Responsibilities

  • Plans, organizes, and directs the activities of the RN- SPECIAL NEEDS, ensuring compliance with legal and regulatory requirements, and customer needs.
  • Serves as the RN- SPECIAL NEEDS nursing administrator responsible for planning, managing, coordinating, directing and evaluating all aspects of the installation serving Nellis and Creech EFMP-M program operations and administrative functions IAW DoD and Air Force policy, and any subsequent implementing guidance.
  • Responsible for interpreting, implementing, and ensuring compliance with DoD and AF instructions (AFI 140-701, AFI 36-2110, DoDI 1315.19, and AFI 36-3020) and any local policies.
  • Serves as the technical authority for the processing and documentation of EFMP medical enrollment, eligibility and the policies, regulations and procedures.
  • Provides guidance, training and assistance to the Family Member Relocation Clearance Coordinators (FMRCC) and alternates, administrative staff, TRICARE representatives, service members and families, and other concerned parties or agencies.
  • Serves as the technical authority for the processing and documentation of EFMP enrollment/disenrollment eligibility of family members, policies, regulations and procedures.
  • Provides oversight, management, assigns duties, prepares performance standards, evaluates work, and/or provides, arranges for training to all EFMP personnel.
  • Guides the patient and family/care giver through the EFMP process, healthcare system, maximizing use of resources.
  • Coordinates and executes care coordination, case management techniques, optimizing access to appropriate medical, behavioral and educational services specific to the program enrollment.
  • Coordinates and functions as a liaison between ancillary services, organizations and agencies to assure medical care services are available to meet the needs of the enrolled family member.
  • Serves as an advocate for, and ensures education is provided to, the patient and family/caregiver as required.
  • Promotes adherence to program requirements for improved healthcare outcomes, services, and support resources utilizing the nursing process.
  • Establishes and maintains procedures to identify sponsors whose family members have special medical and/or education conditions.
  • Conducts assessments and screenings of family members of active-duty sponsors, to determine if special medical and/or education conditions exist that require EFMP enrollment.
  • Assists identified families obtain information on required services available within the community, is a liaison for services and support available through EFMP- Family Support, and ensures access to necessary medical, behavioral and educational services upon reassignment.
  • Identifies and clarifies issues/concerns and determines appropriate referrals with medical, education, ancillary services, and TRICARE, AFPC, and general community on/off the instillation.
  • Provides assistance, care coordination, case management, guidance and advice in prioritization of developing plans and goals tailored to specific conditions or concerns.
  • Provides routine and annual follow up assistance with referral options ensuring access and assistance in securing cost-effective support or direct aid is meeting individual family needs.
  • Monitors and tracks each case to evaluate for ongoing enrollment eligibility and takes action to assist families when no longer require to be enrolled in EFMP-M.
  • Performs periodic case reviews recommending and carrying out appropriate actions to resolve concerns.
  • Appropriate review and documentation of care in medical record and designated databases with update of services needed and collaboration with appropriate agencies.
  • Develops and maintains effective working relationship EFMP-FS, EFMP-A, AFMRA, TOPA, Medical Management, clinics and other agencies in the delivery of available services and/or resources.
  • Assists families with all aspects of EFMP-M program to include enrollment, disenrollment, courtesy clearances, and travel clearance.
  • Has close working relationship with the Military Personnel Section (MPS) in updating Assignment Limitation Code (ALC) “Q” designation is placed in AFPC’s database of AF personnel when a sponsor has one or more family members that meet(s) DoD criteria for EFMP.
  • Develops and maintains up-to-date databases on EFMP enrollees and oversees maintenance of EFMP records.
  • Notes essential information such as demographics, family member with identified need, type of need, etc.
  • Transfers and closes necessary cases as family PCS, separates or retires from the Air Force, or the special need(s) no longer exist.
  • Ensure tracing system is developed for incoming and out-going family members.
  • Requests and maintains via MPS necessary desired lists or rosters for families with identified Q-Code.
  • Ensures MPS is notified of new enrollee’s determination of ALC Q-Code for AFPC notification and update.
  • Utilizes various computer software programs in researching, networking and/or exchanging information and establishing files (e.g. Q-Base, Word, Excel, and PowerPoint).
  • Maintains statistical data for workload accountability.
  • Designs and establishes marketing strategy and public relations programs to ensure target population is informed of services and programs.
  • Prepares and delivers briefings on services and programs directly impacting family members enrolled in EFMP.
  • Collaborates with installation agencies to provide briefings tonew and existing families and personnel on the EFMP program and services offered.
  • Briefs key personnel and leadership on trends and issues impacting service members.
  • Establishes, maintains and updates services and resource materials.
  • Ensure appropriate interfacing with the Integrated Delivery System (IDS).
  • Establishes and maintains good interpersonal relationships with co-workers, families, peers, and other health team members.
  • Completes orientation and competency verification programs in accordance with (IAW) hospital guidelines.
  • Ensures a safe work environment, employee safe work habits and patient safety IAW regulatory agencies, infection control policies, and process improvement initiatives.
