About The Position

This community nursing role involves scheduled visits to care for patients of all ages with varying complexity of care needs in various settings, with the goal of helping patients remain comfortable in their homes for as long as possible. The position offers autonomy, independence, and a work-life balance, supported by a team of peer nurses and supervisors. It also provides opportunities to develop long-term therapeutic relationships with patients, access to best-in-class training and development programs, and celebration and recognition programs.

Requirements

Registration with the CNO as a Registered Nurse in good standing
Valid driver’s license and access to a vehicle is required
Reliability and flexibility
Willingness to travel to and from patient residences within a designated area
Passion for making a difference in your community

Responsibilities

Using appropriate assessment tools/techniques to assess patients’ individualized needs
Executing and continuously monitoring care plans
Engaging patients using a collaborative approach by building rapport and trust
Maintaining and updating professional patient records related to patient encounters and plans
Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
