About The Position
Oak Park Oasis is seeking a Registered Nurse to provide high-quality, patient-centered care at its facility in Oak Park, Illinois. The RN works closely with a multidisciplinary healthcare team to care for patients with diverse needs, including complex medical conditions. The facility aims to reduce rehospitalizations and help patients regain their optimal health, physical function, and independence in a safe, supportive environment.
Responsibilities
- Provide individualized patient care and assess patients with diverse needs, including complex medical conditions.
- Develop and implement patient care plans in collaboration with the care team, physician, and family.
- Monitor patient progress, update care plans, and communicate patient status to the care team and family.
- Coordinate with healthcare professionals and community resources to support continuity of care and seamless transitions.
- Participate in staff development and education, and stay current with industry trends and best practices.
- Collaborate with the medical director to develop and implement evidence-based care practices.
