This Per Diem On-Call Weekends Hospice RN role is field-based in the San Gabriel Valley areas, offering $50/hour plus mileage reimbursement. This role is about changing lives at the most meaningful moments by guiding patients and families through life's vulnerable transitions, delivering comfort, dignity, and compassionate care at home. Requirements
- Licensed Registered Nurse (RN)
Nice To Haves
- BSN/MSN
- Experienced in hospice, home health, or case management
- Compassionate, independent, and confident in the field
- Comfortable with EMR/documentation
Responsibilities
- Lead and coordinate individualized hospice care plans
- Provide skilled nursing care including pain and symptom management
- Collaborate with physicians and the interdisciplinary team
