About The Position

Brighton/Suncrest Hospice is seeking a Registered Nurse to provide compassionate, individualized hospice care wherever patients call home, including private residences, skilled nursing facilities, and assisted living communities. The RN collaborates with an interdisciplinary team—including nurses, Social Workers, Chaplains, CNAs, and volunteers—to develop care plans supporting the physical, emotional, and spiritual needs of patients and their families. Lower clinician caseloads and higher staff-to-patient ratios allow nurses to spend meaningful time with patients and families and provide personalized care. Requirements

• Current Registered Nurse (RN) license in good standing in the state of practice.
• At least 1 year of hospice experience.
• Commitment to exceptional, patient-centered hospice care.
• Strong collaboration and communication skills with interdisciplinary care teams.
• Ability to work independently and manage a daily schedule.
• Excellent time-management and organizational skills.
• Comfort learning and using electronic medical record (EMR) systems.
• Reliable transportation, a valid driver’s license, and current auto insurance for travel throughout the service area.

Responsibilities

• Provide individualized hospice care in patients’ homes, skilled nursing facilities, and assisted living communities.
• Collaborate with nurses, Social Workers, Chaplains, CNAs, and volunteers to develop care plans for patients’ physical, emotional, and spiritual needs.
• Manage daily schedules, patient visits, documentation, meetings, and urgent needs.
